Representative Payee

We strive to provide a peace of mind for those in need and their families.
Living Choice Program Provider
Oklahoma’s Living Choice Program assists eligible individuals in transitioning from institutional settings, such as nursing homes, back into their own homes or community-based settings. This initiative aims to promote community-based services over institutional care, enhancing participants’ independence and quality of life.
Eligibility Criteria:
• Aged 19 or older.
• Have a physical disability (ages 19-64) or are aged 65 and older.
• Have resided in an institutional setting for at least 60 consecutive days.
• Qualify for SoonerCare (Oklahoma’s Medicaid program) for at least one day prior to transition.
• Express a voluntary desire to transition back into the community.
• Be willing to actively participate in their plan of care.
Services Offered:
• Assistance in finding a new residence.
• Home-delivered meals.
• Transportation services.
• Skilled nursing care.
• Therapy services (physical, occupational, and speech).
• Personal care assistance.
• Medication management.
• Transitional funds (a one-time benefit of up to $2,400) to facilitate the move.
Advantage Waiver Program Provider
The Advantage Waiver Program offers alternatives to nursing facility placement for eligible adults, promoting independence and community living.
Eligibility Criteria:
- Age and Disability: Applicants must be 65 or older, or adults aged 21 and above with physical disabilities. Individuals with intellectual disabilities or cognitive impairments are not eligible.
- Financial Requirements: Must qualify for SoonerCare (Oklahoma’s Medicaid program).
- Medical Necessity: Must meet the nursing facility level of care as determined by the Aging Services Division.
- Residency: Must reside in their own home or a family member’s home.
- Support System: Needs must be safely met with waiver services and available family or community supports.

Mental & Behavioral Health Services
We offer a range of mental and behavioral health services for individuals experiencing mental illness, substance use disorders, and other behavioral health challenges.
Types of Therapy
- Individual Therapy – One-on-one counseling with a licensed therapist to address personal challenges such as anxiety, depression, trauma, or stress.
- Group Therapy – A therapist-led group where individuals with similar struggles share experiences and coping strategies.
- Family Therapy – Involves family members to improve communication, resolve conflicts, and support loved ones with mental health issues.
- Couples Therapy – Helps partners work through relationship issues, communication problems, or conflicts.
- Substance Use Counseling – Support for addiction and recovery.
- Telehealth Therapy -Telehealth therapy allows individuals to receive mental health counseling and support remotely via video calls, phone calls, or secure online platforms.
